Beginning your journey with BHSSC Developmental Disabilities services starts with a simple first step: reaching out to determine eligibility and connect with the right supports. Our goal is to make the process clear, supportive, and centered around each individual’s unique needs and goals.
BHSSC provides a wide range of services that help individuals with intellectual and developmental disabilities live, work, and participate fully in their communities. These supports may include residential services, employment and day programs, community participation, transportation, and individualized skill-building opportunities designed to promote independence and quality of life.
Getting Started
The admissions process begins by connecting with Dakota at Home, the central point of entry for developmental disabilities services in South Dakota. Dakota at Home staff will help guide individuals and families through the application process, provide the necessary paperwork, and determine eligibility for services.
Once eligibility is confirmed, individuals are connected with a Conflict-Free Case Manager (CFCM). The case manager works closely with the individual and their support network to identify goals, explore available services, and develop a person-centered plan that reflects their preferences, strengths, and needs.
Eligibility Requirements
To qualify for services, individuals must meet state-established criteria, which generally include:
- A documented intellectual or developmental disability diagnosed prior to age 18
- Conditions such as autism, cerebral palsy, Down syndrome, epilepsy, brain injury, or similar developmental disabilities
- Financial eligibility based on SSI or income guidelines established by the state of South Dakota
Choosing Services & Providers
After eligibility is established and a service plan is developed, individuals and families can explore provider options that best meet their needs. BHSSC collaborates with community partners to ensure access to high-quality supports that promote independence, inclusion, and personal growth.
For individuals already receiving services who are relocating or seeking a change in provider, case managers assist with coordination and referrals to ensure a smooth transition.
