Family Support 360 is a flexible, person-centered program designed to help children and adults with intellectual and developmental disabilities live as independently as possible in their homes and communities while staying connected to their families and natural supports.

Through an individualized approach, Family Support 360 gives participants and families more control over the services they receive. Each person works with a dedicated Family Support Service Coordinator who helps identify needs, connect to resources, and build a customized plan that supports everyday living, independence, and personal goals.

The program offers a wide range of supports that can be combined based on what each family needs. These may include respite care, personal care assistance, companion services, supported employment, and help with daily living skills. It can also support accessibility needs such as home modifications, vehicle adaptations, adaptive equipment, and specialized medical supplies.

Family Support 360 is designed to be flexible and responsive, meaning services can adjust as needs change over time. The focus is on strengthening families, supporting choice, and helping individuals remain active participants in their communities while receiving the right level of support at home.
